In connection with your volunteering/employment with Texas Amateur Athletic Federation Affiliates CPO (the “Company”), notice is hereby given that a consumer report and/or investigative consumer report may be obtained from a consumer reporting agency for volunteer/employment purposes. These reports may contain information about your character, general reputation, personal characteristics and mode of living, whichever are applicable. They may involve personal interviews with sources such as your neighbors, friends or associates. The reports may also contain information about you relating to your criminal history, driving and/or motor vehicle records, education or employment history, or other background checks. The scope of this notice and below authorization is not limited to the present and will continue throughout the course of your volunteering/employment and allow the Company to conduct future screenings for retention, promotion or reassignment, as permitted by law and unless revoked by you in writing. You have the right, upon written request made within a reasonable time after the receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report prepared by contacting the Consumer Reporting Agency. For information about the Consumer Reporting Agency’s privacy practices, please reference the contact information located at the bottom of this form. DISQUALIFYING CRIMES If a coach: (1) has been convicted of, (2) has a charge pending against him or her in which it is alleged that he or she has committed any of the following crimes, or (3) has a record of a conviction of an equivalent offense in another state, the coach will be disqualified from the position with any program approved by TAAF, or (4) multiple felonies and or misdemeanors over multiple DATES, or (5) pleads guilty to
Examples include, but are not limited to: child molestation, rape, sexual assault, sexual battery, sodomy, prostitution, solicitation, indecent exposure, etc.
Examples include, but are not limited to: murder, manslaughter, aggravated assault, kidnapping, robbery, aggravated burglary, etc.
Examples include, but are not limited to: drug offenses, theft, embezzlement, fraud, child endangerment, etc.
Examples include, but are not limited to: simple assault, battery, domestic violence, hit and run, etc.
Examples include, but are not limited to: driving under the influence, drunk and disorderly, public intoxication, etc.
Examples include, but are not limited to: simple drug possession, possession of drug paraphernalia, etc.
Examples include, but are not limited to: contributing to the delinquency of a minor, providing alcohol to a minor, etc. Acknowledgement and Authorization By signing below I hereby authorize the obtaining of consumer reports and/or investigative consumer reports by the Company at any time after receipt of this authorization and throughout the course of my volunteering/employment, if applicable. I also hereby authorize that I have read and fully understand the TAAF Pass/Fail Criteria for background checks for 2024.
